  These are the forms you will need to complete to take this class.

     If this is the first time you are enrolling for a class at PDCCC, complete all three forms. Click on the form link and it will open in Acrobat Reader. Print the form, complete it and deliver it to the Workforce Center (either by hand to 100 N. College Drive, Franklin; by mail at 100 N. College Drive, Franklin, VA 23851 or fax it to 757-569-6055.)

     If you have taken credit classes prior to this, you simply complete Form 3, the Class Selection Form. Please print it and complete it. If you have received an Employee ID (EMPLID) for a class taken earlier, you only have to complete your name and that code. If you do not have that ID, you must provide your Social Security Number.
